Steps to Co-Management

Counsel Your Patient
Review your patient’s specific needs and discuss the option of having a refractive or cataract surgery consultation at Herzig Eye Institute. Refer your patient to us to initiate the co-management process.

Consultation
We will conduct a thorough consultation, utilizing our state-of-the-art diagnostics. Our doctors will review the results and recommend a personalized procedure to meet the patient’s vision goals.

Procedure
Your patient will be in the care of our highly trained and dedicated staff on the day of their procedure. Post-operative care will be scheduled at your clinic, and you will receive a surgery report.

Post-Operative Care
When your patient has been medically cleared to return to your practice for follow up care, you will receive a report with the surgical results and post- operative guidelines. The post-operative exam schedule varies, depending on the procedure.

Co-Management
After each post-operative appointment, fax the completed post-operative or data collection report to us. You will be reimbursed according to our co-management guidelines.
